WAIVER, RELEASE, AND ASSUMPTION OF RISK FORM
On behalf of myself, my household members, and any minor child(ren), I hereby give permission for my child to attend the event noted in the event registration or verify that I am attending the event at The Bible Chapel (any campus). My child and I are familiar with, and knowingly and voluntarily accept, all risks associated with attending the event at the campus noted in the event registration. I acknowledge our participation in this program is wholly voluntary.
I specifically assume all risks and hazards associated with our participation. I understand that we will be associating with staff, volunteers, and other children and may contact viruses (including COVID-19) and diseases through our participation.
While instruction and reasonable supervision will be provided, staff, and volunteers cannot ensure our safety. Accidents and injuries happen, and it is impossible to eliminate the risk that we will suffer an injury or illness.
I certify that we are in good health, have no fever, and have no current issues that make it unsafe for us to participate in the event, which may or may not have a medical professional on-site. I acknowledge that my child and I are responsible for ensuring that he or she takes any necessary medication, and for avoiding any allergies. In the event of a medical emergency, 911 will be called and I will be responsible for all costs of medical treatment.
By signing this form, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of others, and assume full responsibility for our participation at The Bible Chapel.
PHOTO RELEASE: I hereby agree on behalf of myself and my child to authorize The Bible Chapel to use any photographs, videos, and electronic recordings of me and or my child(ren) for Bible Chapel promotional purposes.
MEDICATION DISTRIBUTION: I give my consent for the emergency medication prescribed to be self-administered by my child, and/or assisted by an adult at the event as needed. I understand that volunteers will make every attempt to reach the parent if these medications are administered and that if an Epi Pen/Auvi-Q is administered, 911 will be called for EMS response.
